The DDS Foundation




Building hope for tomorrow for people with disabilities in Lebanon County.
The mission of Developmental & Disability Services (DDS) of Lebanon Valley Foundation, Inc., is to fund programs for the training and education of children with developmental delays and adults with disabilities living in Lebanon County.
The purpose of the DDS Foundation is to raise, manage and distribute funds for programs within Lebanon County that provide training, education and therapy for persons with disabilities.  Based on the availability of funds, grants are awarded each year to organizations that qualify as exempt under Section 501(c)3 of the Internal Revenue Code.

Developmental & Disability Services of Lebanon Valley Foundation, Inc. is a 501(c)3 Charitable Organization. Donations to which are tax deductible to the fullest extent permitted by law. A copy of the official registration & financial information for Developmental & Disability Services of Lebanon Valley Foundation, Inc. may be obtained from the Pennsylvania Department of State by calling toll free from within Pennsylvania, 1-800-732-0999.

Governance:

The Foundation was created exclusively for charitable, educational and scientific purposes. As a charitable 501(c)3 orgnaization, donations are tax deductible to the fullest extent of the law.  A copy of the official registration may be obtained by calling within Pennsylvania 1-800-732-0999. Governed by a volunteer five member board of directors, which is comprised of community members and business leaders who are sensitive to the needs of people with disabilities, the DDS Foundation is managed by one part time employee.

Who Benefits:

The main benefactor of the DDS Foundation is Developmental & Disability Services (DDS) of Lebanon Valley. The DDS agency has been in existence since 1961. The Foundation was formed in 1997 to ensure funding for the agency. Fundraising for the Foundation is actively supported by DDS board directors, employees, volunteers and participants.

Giving Opportunities:

Gifts may be made to the DDS Foundation, Inc. in the form of:
• Outright gifts of cash, stocks and/or bonds
• Memorials
• Real estate
• Donor restricted funds
• Endowment/Heritage Club
• Naming the DDS Foundation as a beneficiary in a will or estate plan
• Naming the DDS Foundation as a beneficiary of a life insurance policy
• Charitable Gift Annuities

Grant Guidelines:

The DDS Foundation accepts applications throughout the year.
The board of directors meets quarterly and will review fund requests in the time frame that they are received.
Applicants must be a 501(c)3 organization and must provide one or more of the following services to persons with disabilities: education, training or therapy.
Applicants should include a detailed statement if grant funds will be used to benefit persons with down syndrome. It should be understood that the Foundation is not limited to providing funds to people with down syndrom, but rather supports all people with disabilities.
Recipients must expend the Foundation grant award within Lebanon County in support or furtherance of its services to the county’s disabled citizens.
The DDS Foundation will not make grants to individuals, or other grant making organizations, or to national, statewide or regional organizations that only incidentally benefit Lebanon County citizens.
Application Procedures:
Organizations seeking a grant from the DDS Foundation must make a written submission to the Foundation at:

The applicant should include the following:
• A description of the organization, its mission and programs.
• A description of the project or program to be funded, a project budget, other funding sources and the targeted population to be served.
• A statement of wether or not person's with Down Syndrom will benefit from the project. If so, how many and how much of the requested funds will benefit htese individuals.
A copy of the organization’s current IRS exempt status of determination letter.
• A current List of the organization’s Board of Directors
• Please submit one original and five copies of the request letter.
• Applicants will be informed in writing as to whether or not their fund request has been granted.
